Position Overview

iWant Wellness Psychotherapy is actively seeking a devoted and skilled Licensed Clinical Social Worker (LCSW) to join our vibrant multidisciplinary care team. This position plays a vital role in advancing our mission to enhance health outcomes for patients with complex medical, behavioral health, and psychosocial needs. By providing trauma-informed care and comprehensive case management, you will help us address health inequities faced by some of the most vulnerable populations in Northern Cambria, Pennsylvania.

You will engage in electronic health record management and utilize advanced telehealth platforms to ensure effective service delivery. Key Responsibilities
  • Deliver intensive case management services to a focused caseload of children and families, ensuring personalized support tailored to their unique circumstances.
  • Triage and effectively respond to varying care needs and tasks, prioritizing according to the urgent requirements of patients.
  • Assist families in completing necessary forms and applications for social services and healthcare resources, ensuring a smooth navigation experience.
  • Utilize assessment skills to address social determinants of health (SDOH) and develop individualized care plans.
  • Facilitate connections for families with pertinent community resources and support systems, such as housing, education, and mental health services.
  • Monitor and track referrals and follow up with families to guarantee service delivery effectiveness.
  • Help patients in scheduling and attending essential medical, behavioral health, and specialist appointments, ensuring they receive comprehensive care.
  • Provide administrative support, including records retrieval and review, eligibility screening, and goal-tracking processes.
  • Coordinate all necessary care by advocating for patients and ensuring all aspects of their health needs are met seamlessly.
  • Conduct home and community visits as needed to provide thorough support and evaluation of the living conditions affecting patient care.
  • Engage with an interdisciplinary care team, including behavioral health specialists, nursing staff, and external providers, to create a cohesive care strategy.
  • Document patient interactions and services meticulously using our electronic health records system and secondary database.
  • Participate in internal and external meetings, including case conferencing and administrative discussions, to improve care delivery.
  • Attend ongoing training sessions on health coaching, engagement techniques, and other relevant topics to enhance clinical skills.
  • Provide timely and accurate reports to the management team on data and outcomes related to Care Management programs.
  • Offer thoughtful insights regarding the implementation of case management services and contribute to continuous quality improvement initiatives.
Patient Population / Client Focus

The LCSW will primarily work with children and families within underserved communities in Northern Cambria.
