Job description

Description Mt San Antonio Gardens is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to support our Independent Living residents. This role is responsible for responding to resident calls, handling campus-wide emergencies, and ensuring continuity of care for residents' medical needs. The LVN plays a key role in maintaining resident safety, coordinating care, and delivering high-quality nursing services in a senior living community. Schedule Morning Shift 7 a.m. - 3 p.m. Responsibilities 1. Checks on report from Out Calls night nurse. 2. Reads log book and emails daily and follows through appropriately. 3. Sets up and assists residents with routine medication. Home Health Aides/Med Techs may assist the residents with medications upon discretion of LVN. 4. Makes house calls as needed and provides routine nursing treatments and administers first aid to the residents 5. Reads all consultation sheets after a doctor's visit and follows through with any new orders. 6. Administers injections and immunizations to residents and employees as needed. 7. Completes Documentation: a. House Calls - completes electronic documentation in progress notes b. Clinic Visits - should be checked on log sheet and electronic documentation done as applies.. c. Immunizations: Document on appropriate forms and progress notes. d. Incidents - Complete Incident reports, Investigation Summary and progress notes. 8. Calls primary physician as needed and reports incidents and concerns you may have for residents. 9. Obtains physicians orders and follows through appropriately. 10. All treatments and medications administered should have a physician's order including first aid care. 11. Coordinates the transferring of independent residents to and from the Villas, Magnolia Grove, hospital, or Transitional Care Room (TCR - Oak Tree Lodge (OTL). 12. Assesses and coordinates the residents move to higher levels of care or hospital as the condition presents itself and notifies responsible parties when transfer occurs. 13. Assesses residents' condition and needs within one (1) hour of discharge from higher levels of care or hospital to their independent living unit; will also complete proper documentation in a timely manner. 14. Order trays if necessary. (Ext.238) 15. Collaborates with AL and Skilled Nursing staff on IL resident's care and needs during temporary stays in these areas. 16. Assesses employee injuries and follows through appropriately per MSAG guidelines 17. Responsible for the supervision of the Home Health Department: a. Monitor daily general routine care provided for the residents. b. Home Health Aide to be available to be in the Clinic area to assist LVN. 18. Maintains supplies for the Clinic. 19. Completes Relias training in a timely manner 20. Attend departmental meetings. 21. Reviews policy and procedure book frequently. 22. Faxes new prescriptions and renewal prescriptions to currently contracted pharmacy. 23. Assures that all medication received from the currently contracted pharmacy is checked in, initialed and dated when received. 24. Ensure delivery of prescriptions from pharmacy to residents, as soon as possible after delivery to the Clinic. 25. Assures that all medication is stored appropriately and locked up in the medicine cupboard or the refrigerator in the Med Room. 26. Is knowledgeable of standing Doctor's Orders (posted on door in the Clinic) 27. Forwards copies of Durable Power of Attorney for Healthcare (DPAHC), Do Not Resuscitate (DNR), Physician's Orders for Life Sustaining Treatments (POLST), Advocacy information (if on file) and emergency sheets when residents are admitted to the acute hospital, AL or Skilled Nursing 28. Reviews medication information and forward to AL or Skilled Nursing. 29. Checks on residents in the hospital in the absence of Skilled Nursing Admissions Coordinator. 30. Assists with procedures in the Residential Health Services Clinic as needed. 31. Knowledgeable of the use of diagnostic equipment in MSAG Physician's Clinic 32. Completes other duties as assigned by supervisor. 33. Practices good safety habits. *** PRIORITY IS ALWAYS ANY EMERGENCY CALL - TAKE SECURITY OR A CERTIFIED NURSING ASSISTANT WITH YOU IF IT IS APPROPRIATE. Requirements
  • California LVN license with two to three years' experience preferred
  • Valid CA Driver's license
  • Knowledge of Geriatric Nursing care
  • Ability to maintain effective working relationships with doctors, residents and employees
  • Ability to maintain professional relationships with residents and not discuss personal problems.
  • Maintain confidentiality at all times.
