Job Summary

The LPN provides nursing services and health care to Grafton clients. Performs a variety of nursing duties associated with monitoring clientโ€™s health and general wellbeing, managing health issues and providing health care. Report to nurse manager.

Essential Job Functionsย ย 

  • Assesses and evaluates clients; obtains and reviews pertinent medical data; collects vital signs and specimens; monitors clientโ€™s response to treatment.ย 
  • Follows through with treatment plans while assisting RNs or physicians in providing direct care to patientsย 
  • Assists with psychological reviews; schedules client appointments with health care providers and dentists. Attends all neurology/specialty appointments, as appropriateย 
  • Documents the administration of care in the client medical record in a timely and thorough mannerย 
  • Provides the medical perspective into client care for all Multidisciplinary Team (MDT) meetings.ย 
  • Administers medication as directedย 
  • Serves as a resource to department staff and students and to help deliver quality client careย 
  • Provides crisis intervention services as necessary.ย 
  • Provides hepatitis B or flu vaccines and PPD testing to Grafton staff.ย 
  • Promotes and assists in the control of communicable disease.ย 
  • Prepares and/or completes various health services forms like medication permission forms, on-call sheets, medical/dentist appointment sheets, etc.ย 
  • Works on-call duty in rotation, as appropriate; promptly responds to calls for immediate nursing services.ย 
  • Attends meetings, seminars and training as required.ย 
  • Performs other work as assigned.

Minimum Qualificationsย 

  • Requires a high school diploma or equivalent. Associates degree preferred.
  • Valid driver's license
  • Valid LPN License from the State Board of Nursing
  • 1 year or moderate experience as a practical nurse in a behavioral health setting, or equivalent combination of education and experience; previous medication administrationย ย 
  • Thorough knowledge of practical nursing techniques
  • Computer proficiency, strong communication skills, ability to maintain effective working relationships with clients and other employees.ย 

COMPENSATION

position may be filled as LPN or RN

  • RN starting at $38.46 up to $51.39/hour; commensurate with experience and qualifications.ย 
  • LPN starting at $30 up to $41.23/hour; commensurate with experience and qualifications.
