Job Title: Clinical Manager - IHSS & Home Health

Job Overview:

We're seeking a dedicated and experienced Clinical Manager to oversee our IHSS and Home Health programs. We are a small agency looking to expand our clinical services. The ideal candidate will exhibit strong leadership skills, a commitment to quality patient care, and a comprehensive understanding of both In-Home Supportive Services (IHSS) and home health care systems. This role is crucial in ensuring our services meet the highest standards and comply with relevant healthcare regulations.

Key Responsibilities:

- Oversee the daily operations of IHSS and Home Health programs, ensuring exceptional patient care and service delivery.
- Lead and mentor Health Maintenance Attendants and other clinical staff, providing guidance and support to maintain and      enhance professional competency.
- Develop and implement policies and procedures that comply with state and federal regulations.
- Collaborate with healthcare professionals, patients, and families to develop personalized care plans that meet patient needs.
- Monitor and evaluate the effectiveness of patient care services, making necessary adjustments to improve outcomes.
- Stay informed of the latest developments in home health and IHSS practices and incorporate them into practice as appropriate.
- Support a positive work environment by promoting teamwork and open communication.

Qualifications:

- Active Registered Nurse (RN) Colorado license required;

- Minimum of 2 years of experience in IHSS, home health, or a related healthcare setting required;

- Minimum of 1 year of supervisory experience in IHSS, home health, or related healthcare setting required;

- Bachelor’s degree in Nursing, Healthcare Administration, or a related field preferred;

- Strong organizational and problem-solving skills with a keen attention to detail;

- Excellent communication and interpersonal skills to foster patient and team relationships;

- Proficiency with electronic health records and healthcare management software.

Working Conditions:

- Full-time position, with on-call availability after hours.
- Role requires travel to client homes or meetings as part of program oversight.
