About ThedaCare!
Summary :
The Clinical Pharmacist Team Leader provides leadership and expertise in professional practice, education, accreditation, and drug policy. Responsible for cost-effective utilization of staff and other resources while maintaining high standards of quality, service, and satisfaction. Supports informatics related issues. Works in collaboration with the care team to interpret and evaluate prescribed medications using drug therapy management principles. Working within the State of Wisconsin pharmacy scope of practice, the pharmacist monitors the procurement, storage, distribution, and disposal of medications.
Job Description:
KEY ACCOUNTABILITIES:
  • Holds pharmacy license at respective site when applicable.
  • Schedules staff and manages daily operational and clinical workflows including the oversight of personnel at the discretion of the manager.
  • Supports ThedaCare drug shortages specifically related to site issues.
  • As the DEA license holder, is responsible for oversight of controlled substances.
  • Represents pharmacy at P&T and other committees.
  • Oversees legal and regulatory compliance at local hospital level including medication error reports, Joint Commission Medication Management standards, and State Pharmacy Board.
  • Applies critical thinking to assess a patient's medications for appropriateness and safety.
  • Utilizes evidence-based practice to ensure expected patient outcomes.
  • Communicates and collaborates within a multidisciplinary team to progress the plan of care across the care continuum.
  • Incorporates quality and safety into a patient's individualized care plan.
  • Collaborates with and supports care team in meeting the desired patient and family outcomes by reinforcing education identified in the patient's plan of care.
  • Is expected to maintain specialized knowledge when and if practicing in a unique clinical practice setting such as oncology, home infusion, and others.
  • Demonstrates professional and therapeutic relationships with patients, families, and care team.
  • Participates in quality and process improvement, committee work, and other projects as assigned to meet the needs of the department and ThedaCare.
QUALIFICATIONS:
  • Bachelor of Science degree or Doctor of Pharmacy degree from an ACPE accredited school of pharmacy
  • American Heart Association Healthcare Provider Basic Life Support (BLS) and American Heart Association Advanced Cardiac Life Support (ACLS) except for ThedaCare At Home, Oncology and Ambulatory Pharmacists
  • Licensure: Active Wisconsin pharmacist license
WORK ENVIRONMENT:
  • Climate controlled office setting with daily movement throughout the hospital campus
  • Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of fifty (50) pounds without assistance
  • Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
  • Normally works in climate controlled office environment
  • Frequent sitting with movement throughout office space
  • Frequent exposure to sharp objects and instruments
  • Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock.
  • Possible exposure to communicable diseases, hazardous materials, and pharmacological agents.
  • Occasional contact with aggressive and or combative patients.
  • Use of computers throughout the work day
  • Frequent use of keyboard with repetitive motion of hands, wrists, and fingers

Position requires compliance with department specific competencies.
