Job description

Position Overview Under general supervision is responsible for providing case management and/or treatment according to established policies and procedures. The individual will assist in creating and fostering an environment conducive to the treatment, operational and administrative goals and mission of MonDay. Summary of Job Duties Essential Duties and Responsibilities Case Management Sets appointments, completes transportation requests, visitor lists, furlough verifications and obtains court orders Determines if residents have their social security card, birth certificate and state ID/driver license Assists with obtaining above mentioned documentation as needed Investigates and set up court dates for residents with warrants Assists in the facilitation with classes to support when clinical staff are out of the office Provides transportation as necessary Organizes and document notes according to standards Assists staff in treatment and aftercare planning including appropriate linkages and progress of the offender while in the program in meeting target behaviors, goals and recommendations Provides case management services by coordinating with other agencies, resources, and professionals within a planned framework of action toward the achievement of established goals Responds to crisis situations and uses appropriate crisis intervention techniques Follows the standardized process to periodically and objectively assess resident progress in meeting case plan goals through established defined criteria Related Programming Interfaces with staff and residents Assists the clinical staff with case management concerns Documents all case management information in the progress and other client related data Operations Follows and enforces Behavior Management System for managing residents Ensures that necessary documentation is completed Assists in monitoring residents and transportation Performs other operational duties as needed Administrative Participates in weekly team meetings, treatment team meetings, and all staff meetings Participates in the development of MonDay policy and procedures Performs public relations with all personnel and the community Obtains annual training as required Performs other duties as required Quality Assurance Audits resident records for currency, completeness, and accuracy Working Conditions Potential exists of exposure to disease/illness.

Minimum Qualifications and Requirements Qualifications High School diploma or GED with 5 years' experience and current Chemical Dependency Counselor Assistant or LSW Assistant required. Associate's degree preferred in a behavioral science in counseling, social work, or a related field and able to obtain a Chemical Dependency Counselor Assistant or LSW Assistant within ninety (90) days of appointment to position.

Must have 40 hours of Chemical Dependency specific education Must maintain certification requirements of the Ohio Chemical Dependency Board or the State of Ohio Counselor and Social Work Board, as appropriate Valid State of Ohio Driver's License with a good driving record required Skills require the ability to analyze, plan, and implement case management concerns which may include coaching, counseling, and positive reinforcement Ability to read and write routine reports, statistical information Interpersonal communication skills requiring ordinary courtesy and tact are necessary to communicate effectively with staff, clients, families, social services, and criminal justice agencies, as well as the general public. Ability to complete and follow through with case management concerns Ability to facilitate groups A working knowledge of the general principles of social work, sociology, psychology, criminal justice, or associated curriculum Familiarity with Cognitive Behavior and Motivational Interviewing and a working knowledge of support group programs Familiarity with standard office software operating systems and programs such as Microsoft Office, Microsoft Word and Excel.
