About the Role

aFit Staffing is seeking an experienced RHTP Grant Manager to support the Rural Health Transformation Program (RHTP). This is a highly visible leadership opportunity focused on strengthening rural healthcare systems and ensuring responsible stewardship of grant-funded resources throughout Indiana.

The Grant Manager will provide strategic oversight of grant compliance, monitoring activities, financial stewardship, and performance reporting across multiple sub-awardees. This role will work closely with program, finance, and data teams to ensure all state and federal requirements are met while supporting long-term rural health transformation goals.

In addition to serving as a key resource for division leadership, this position will supervise Grant Analysts and help build sustainable monitoring processes that drive accountability, transparency, and measurable outcomes across the initiative.

What You'll Do Program Leadership & Compliance
  • Lead implementation of risk-based monitoring systems for sub-awardee compliance.
  • Oversee desk, virtual, and on-site monitoring activities.
  • Interpret state and federal grant requirements and ensure adherence across funded partners.
  • Develop and monitor corrective action plans to address compliance deficiencies.
  • Maintain audit-ready documentation and support state and federal audits.
Financial Stewardship & Data Oversight
  • Review complex financial documentation and validate allowable expenditures.
  • Oversee sub-award spending utilizing PeopleSoft Financials.
  • Evaluate reporting data for accuracy, completeness, and performance alignment.
  • Identify financial and operational risks and recommend mitigation strategies.
Stakeholder Engagement & Continuous Improvement
  • Serve as a strategic advisor on grant, finance, and administrative matters.
  • Provide technical guidance and training to sub-awardees.
  • Collaborate with program, finance, and data teams to resolve issues and improve program performance.
  • Contribute to enhancements of monitoring tools, templates, and procedures.
Team Leadership & Collaboration
  • Supervise RHTP Grant Analyst team members.
  • Promote accountability, collaboration, and consistent grant management practices.
What You Bring Required Qualifications
  • 4-6 years of experience in grant administration, program evaluation, quality assurance, data analysis, or data management.
  • Knowledge of federal and state laws governing financial and programmatic activities.
  • Working knowledge of business administration, accounting principles, and grant management.
  • Experience reviewing financial data and managing compliance-related activities.
  • Strong analytical, problem-solving, written, and verbal communication skills.
  • Proficiency with Microsoft Office and PeopleSoft Financials.
  • Demonstrated leadership or supervisory experience.
Highly Desired
  • Experience supporting CMS-funded programs or federal cooperative agreements.
  • Familiarity with Indiana's rural health landscape.
  • Experience with reporting systems such as RedCap, EUNA, or Amplifund.
Work Environment
  • Hybrid work arrangement with 2-3 days in office.
  • Full-time assignment, 37.5 hours per week.
  • Long-term opportunity supporting a five-year grant initiative.
Why This Role Matters

This position offers the opportunity to directly influence healthcare transformation efforts across rural Indiana communities. As part of a long-term statewide initiative, the RHTP Grant Manager will help ensure funding is managed responsibly, compliance standards are met, and meaningful health outcomes are achieved for underserved populations throughout the state. This role provides significant visibility within IDOH and the opportunity to make a lasting impact on Indiana's healthcare future.
