As a VSP Nurse Practitioner at CenterWell Senior Primary Care, you'll be part of a collaborative team. This team includes other APP's, Pharmacists, Care Coaches, Behavioral Health Specialists, and more-all working together to deliver personalized, high-quality care.
Reporting to our Associate Medical Director, you will be part of a team dedicated to supporting and empowering clinicians to do what they love. Our leadership is committed to promoting an environment where you will provide the highest quality care and establish relationships with those you serve.

Your Role & Responsibilities
  • Provide comprehensive Primary Care to seniors
  • Collaborate daily with interdisciplinary team.
  • Document in Athena EMR.
  • Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time.
  • You will work on-site, in an outpatient office setting

Required Qualifications:
  • Master's degree in nursing with board certification.
  • Active, unrestricted NP license in the state of practice.
  • Minimum two years' Nurse Practitioner experience.
  • Commitment to improving patient experience and outcomes.
  • EMR experience and adapts well to new technologies.
  • Participation in Tuberculosis (TB) screening program.

Preferred Qualifications:
  • Experience in Value-Based care, Primary Care or Medicare Advantage settings working with Geriatric patients.
  • Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license or Medicaid and Medicare numbers.
  • Familiarity with HEDIS quality indicators and Medicare guidelines.

Work Environment
  • Outpatient clinical setting.
  • Our clinics are open Monday-Friday 8am-5pm.

Scheduled Weekly Hours
  • Availability for 3 days a week consistently preferred
  • Coverage for CenterWell Fort Worth clinics: Arlington, Grand Prairie, Seminary, Camp Bowie, N. Richland Hills

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
