Registered Nurse (RN) Home Health & Hospice

Job Summary
The Registered Nurse (RN) provides skilled nursing care to patients in their homes under the direction of a physician and according to agency policies and procedures. The RN assesses patient needs, develops care plans, administers treatments, educates patients and families, and coordinates care with the interdisciplinary team to ensure quality patient care and comfort.


For hospice patients, the RN focuses on pain management, symptom control, emotional support, and end-of-life care.

Essential Duties and Responsibilities
Home Health Duties


  • Perform patient assessments and nursing evaluations
  • Develop and update individualized care plans
  • Administer medications and treatments as ordered
  • Monitor patient condition and progress
  • Educate patients and caregivers regarding disease management and safety
  • Coordinate care with physicians, therapists, and other healthcare staff
  • Complete accurate and timely documentation
  • Perform wound care, catheter care, IV therapy, injections, and other skilled nursing procedures
  • Ensure compliance with Medicare, Medicaid, and agency guidelines

Hospice Duties


  • Provide compassionate end-of-life nursing care
  • Manage pain and symptoms for hospice patients
  • Educate families regarding hospice care and patient comfort
  • Provide emotional support to patients and families
  • Coordinate with hospice interdisciplinary team
  • Respond to patient emergencies and changes in condition
  • Assist with bereavement support as appropriate
Qualifications
Education


  • Graduate of an accredited school of nursing
  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Licensure


  • Current and active Registered Nurse (RN) license in the State of Texas

Experience


  • Minimum 1 year nursing experience preferred
  • Home Health and/or Hospice experience preferred
  • Experience with wound care, IV therapy, and patient assessment preferred
