Description

The Pharmacist accurately reviews and interprets medication orders.

ESSENTIAL FUNCTIONS/ RESPONSIBILITIES

  • Ensures compliance with patient care quality standards as it relates to the care provided to all age groups of patients ranging from newborns to the elderly
  • Assists with the purchase and inventory maintenance of pharmaceuticals and related substances and supplies
  • Ability to verify patient orders, modify orders (for example adjust IV dosage times), and enter new orders as requested
  • Handles day-to-day operations within the pharmacy department
  • Monitors activities of Pharmacy Services and takes appropriate action as necessary to ensure efficient and effective operation
  • Works closely with the nursing and medical staffs and other clinical department managers in addressing Pharmacy Services issues as they are related to the services provided to all patients ranging from the newborn to the elderly. Understands that there are dosage variations between newborns, pediatrics, adolescents, adults, and geriatrics and knows where to find appropriate information
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors
  • Supports and maintains a culture of safety and quality
  • Represents the organization in a positive and professional manner
  • Maintains a system of records in accordance with hospital policy for recording patient charges as well as maintaining adequate controls over requisitioning and dispensing of pharmaceuticals 
  • Obtains appropriate hospital-specific education and training when necessary
  • Responds to common inquiries or complaints from customers, regulatory agencies or members of the hospital staff
  • Effectively presents information to management, medical staff, and hospital staff 
  • Complies with all organizational policies regarding ethical business practices
  • Communicates the mission, ethics, and goals of the hospital , as well as the focus statement of the department
  • Ensures that pharmaceutical preparations are sterile, properly packaged, labeled, stored, and not outdated
  • Adherence to dress code, appearance is neat and clean
  • Completes annual education requirements 
  • Promotes the optimal use of antimicrobial agents while minimizing unintended consequences of antimicrobial use.
  • Maintains and ensures patient confidentiality at all times 
  • Ability to verify orders as required after regular pharmacy operating hours using the Pharmacy Remote Order Verification System
  • Interprets physician prescriptions and medication orders - reviews prescriptions issued by physicians and other authorized prescribers to assure accuracy and determines formulas and ingredients needed
  • Monitors all posting onto the patient profiles in order that accuracy, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities may be realized and better patient care received 
  • Directs Pharmacy Services staff engaged in admixing, packaging, and labeling pharmaceuticals
  • Reviews and monitors drug therapy that requires dosing adjustments as authorized by the physician
  • Assists the Pharmacy Director on special assignments
  • Supervises at least 1 or 2 members of Pharmacy Services staff on a daily basis
  • Acts as a drug information resource to patients, medical staff, nursing staff, and ancillary department staff
  • Compounds and dispenses prescribed medications and other pharmaceuticals for patient care by performing the related duties
  • Adheres to all Jackson Parish Hospital policies, procedures, safety plans, and all standards imposed by regulatory organizations
  • Reports to work on time and as scheduled, completes work within designated time
  • Wears identification while on duty 
  • Works at maintaining a good rapport and a cooperative working relationship with physicians, departments, and staff
  • Performs all/any additional duties as assigned by the Pharmacy Manager
  • Reports to work on time and as scheduled; completes work within designated time frames.
  • Attends staff meetings as scheduled.  
  • Represents the organization in a positive and professional manner.

Requirements

EDUCATION, TRAINING, AND EXPERIENCE:

  • Doctorate of Pharmacy degree or Bachelor of Science degree or equivalent
  • Graduate from accredited School of Pharmacy   

LICENSURE/CERTIFICATIONS

  • Current Pharmacist certification or licensure in the State of Louisiana 
  • Membership in The Louisiana Society of Health-System Pharmacists (LSHP) and The American Society of Health-System Pharmacists (ASHP) is preferred 

LANGUAGE and PROFICIENCY REQUIREMENTS:

  • Able to communicate effectively in English, both verbally and in writing. 
  • Strong written and verbal skills.

SKILL REQUIREMENTS:

  • Ability to work alone or with minimal supervision.
  • Ability to work long hours between breaks and meals, if necessary.  
  • Basic computer competency.  

MACHINERY/TOOLS/EQUIPMENT REQUIREMENTS:

  • Ability to operate all pharmacy related equipment, including, but not limited to: Omnicell or other automated pharmacy systems, Cactus, IV Laminar Flow Hood, Auto-Print Unit Dosing System, Morris & Dickson wholesaler website for medication ordering. Also, nursing call system, telephone, fax machine, copy machine, computer, and printer.
