Home Care Intake Clerk

The Home Care Intake Clerk is responsible for managing all aspects of the patient intake process. This includes handling medical records, data entry, maintaining patient records, coordinating telephone calls, and managing insurance verification and authorization processes.

Review and process referrals from hospitals, physicians, patients, or families to determine eligibility and need for home health services, ensuring compliance with federal, state, and agency regulations.

Coordinate with healthcare professionals to obtain patient medical history, documentation, and insurance information necessary for admission and billing purposes.

Schedule initial home visits for the clinical team, ensuring timely and efficient start of care for patients.
