Role overview

Weโ€™re seeking an experienced RN Case Manager for an acute-care hospital setting supporting care coordination and discharge planning for primarily adult patients (including neuro/cardiac populations).

Key responsibilities
  • Coordinate patient care plans across the interdisciplinary team
  • Lead discharge planning, transitions of care, and resource coordination
  • Complete needs assessments and arrange DME/services as appropriate
  • Collaborate with nursing, providers, pharmacy, therapy, social work, etc.
Requirements
  • RN Case Management experience (inpatient/acute care)
  • Epic experience (required)
  • BLS (Basic Life Support)
  • Comfort managing higher caseloads (typical experience range noted: ~18โ€“25 patients)
Unit / workflow details
  • Adult patient population; daily census noted around 165
  • Typical ratios noted:
    • Days: ~20:1
    • Weekends: ~50:1
Weekend / floating expectations
  • Weekend coverage: minimum 2 weekend days per month (typically 1 Saturday + 1 Sunday, not the same weekend)
  • Floating: may float to other local campuses if census is low (within the same health system)
Compliance & onboarding notes
  • References: 2 references covering 1 year within the last 3 years (Charge/Lead accepted)
  • Modules: non-billable; average ~16 hours (20+ hours requires approval)
  • Vaccines: COVID not required; flu status varies/confirm during compliance
