Brookestone Home Health & Hospice is seeking a Full-Time Hospice RN Case Manager to serve Holdrege and the surrounding communities.

As part of the Vetter Senior Living family, Brookestone Home Health & Hospice is dedicated to delivering personalized hospice care in the comfort of our patients' homes. Our Hospice RN Case Managers play an essential role in coordinating care, managing patient needs, supporting families, and working collaboratively with an interdisciplinary team to ensure exceptional end-of-life care.

What You'll Do
  • Coordinate and manage hospice care plans for assigned patients
  • Perform nursing assessments and monitor patient conditions
  • Provide education, guidance, and support to patients and families
