POSITION OVERVIEW:
The RN Case Manager is responsible for the full delivery of the plan of care for each hospice patient, while continuously evaluating personal and professional performance and making necessary changes to increase productivity, and improve the quality of care delivered. A few position highlights include:
  • Make assigned patient care visits in accordance with physician orders. Complete supporting documentation accurately and timely, including visit report, OASIS assessment, plan of care and revisions, medication profile, progress notes, recertification, discharge/transfer summary, etc.
  • Attend meetings for all patients assigned. Participate and offer input regarding each patient's care and progress toward individual patient goals.
  • Understand and adhere to all agency policies, procedures, and processes (administrative, personnel, clinical, etc.). Seek guidance and clarification when uncertainty/confusion arises.
  • Establish and maintain an effective and efficient visit route.
  • Provide services within the scope of practice as defined by the state laws governing the practice of nursing, initiates and coordinates the plan of care, collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily.
THE MUST HAVES:
  • One (1) year of clinical RN experience.
  • Current RN license in the state where services are rendered.
  • Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
