Registered Nurse (RN) – Hospice Case Manager

Location: Ames, IA

Work Type: Field-Based | Full-Time

Compensation: Starting at $48+/hour (Based on Experience)

Schedule

  • Monday–Friday, 8:00 AM – 5:00 PM

  • On-call rotation: One weeknight and one weekend day or night every other week

  • Approximately six patient visits per day

  • Travel within approximately a 60-mile service radius


About the Role

Our client is seeking a compassionate and experienced Hospice Registered Nurse (RN) Case Manager to provide exceptional end-of-life care while serving as the primary clinical advocate for patients and their families.

In this role, you'll manage an assigned caseload, coordinate care across an interdisciplinary hospice team, and ensure every patient receives individualized, high-quality care that honors their goals, comfort, and dignity. This position offers meaningful patient relationships, clinical autonomy, and the opportunity to make a lasting impact during one of life's most important moments.


Key Responsibilities

  • Provide comprehensive nursing care to hospice patients in home and community settings.

  • Complete initial and ongoing patient assessments and update care plans as clinical needs evolve.

  • Develop and implement individualized Hospice Plans of Care in collaboration with physicians and the interdisciplinary care team.

  • Serve as the primary liaison between physicians, patients, families, and hospice team members.

  • Coordinate care with Licensed Practical Nurses, Certified Nursing Assistants, social workers, chaplains, and other support staff.

  • Educate patients and caregivers regarding medications, symptom management, oxygen therapy, wound care, hospice philosophy, and disease progression.

  • Monitor pain management and symptom control while advocating for patient comfort and quality of life.

  • Communicate changes in patient condition promptly to attending and hospice physicians.

  • Maintain complete, accurate, and timely electronic clinical documentation.

  • Conduct routine supervisory visits with assigned team members and support clinical excellence.

  • Participate in Quality Assessment and Performance Improvement (QAPI) initiatives.

  • Follow infection prevention protocols and regulatory standards.

  • Participate in the agency's on-call rotation as scheduled.

  • Attend staff meetings, complete required compliance training, and support continuous quality improvement initiatives.


Required Qualifications

  • Active Registered Nurse (RN) license in the State of Michigan.

  • Previous hospice, home health, palliative care, or related nursing experience.

  • Strong patient assessment and case management skills.

  • Excellent communication and interpersonal abilities.

  • Ability to work independently while collaborating effectively within an interdisciplinary team.

  • Current driver's license, reliable transportation, and ability to travel throughout the assigned territory.

  • Proficiency with electronic medical records and clinical documentation.


Preferred Qualifications

  • Experience using Homecare Homebase (HCHB).

  • Prior hospice case management experience.

  • OASIS documentation experience.

  • Knowledge of Medicare hospice regulations and compliance standards.
