Job description

Job Overview

We are a post‑acute care provider dedicated to holistic, family‑centered end‑of‑life care. As a Hospice, General Inpatient Nurse‑Case Manager, you will deliver compassionate care in our Inpatient Hospice setting.

Key Responsibilities
  • Implement plans of care focusing on comfortable and safe dying, life closure, and effective grieving.
  • Act as a family educator and advocate, maintaining dignity and supporting spiritual and cultural beliefs.
  • Complete all clinical documentation following agency protocol and Medicare/federal guidelines.
  • Provide information to referral sources and coordinate transition from hospital to hospice.
  • Attend interdisciplinary team meetings to discuss patient and family needs.
Required Qualifications
  • Registered Nurse licensed in the state of employment.
  • Bachelor’s Degree in Nursing preferred.
  • Minimum 3 years inpatient clinical experience, preferably hospice.
  • Valid driver’s license and auto insurance.
  • Ability to meet all physical demands of the role.
