Hospice RN

At Granite VNA, hospice nursing is about more than providing clinical care—it's about bringing comfort, dignity, and compassion to patients and their families during one of life's most meaningful journeys. As a Hospice RN, you'll be a trusted guide, advocate, and clinical expert, working alongside an interdisciplinary team dedicated to exceptional end-of-life care.

What You'll Do
  • Build meaningful relationships with patients and families while explaining the hospice philosophy, available services, program expectations, and bereavement support.
  • Complete comprehensive patient assessments and develop individualized Plans of Care in collaboration with physicians, patients, caregivers, and the interdisciplinary team.
  • Deliver skilled nursing care that promotes comfort, symptom management, and quality of life while honoring each patient's goals and wishes.
  • Accurately document patient assessments, interventions, education, progress, and care plans in accordance with agency standards and regulatory requirements.
  • Empower patients and caregivers through education, ensuring they feel confident and supported throughout the hospice journey.
  • Partner closely with physicians, social workers, chaplains, home health aides, volunteers, and other healthcare professionals to provide coordinated, compassionate care.
  • Participate in interdisciplinary team meetings, patient care conferences, quality improvement initiatives, and agency committees to continually enhance patient outcomes.
  • Attend ongoing education and in-service programs to stay current with best practices and continue your professional growth.
  • Travel throughout the Granite VNA service area using your personal vehicle to provide in-home hospice visits and attend team meetings.
Additional Responsibilities
  • Provide clinical supervision and support to Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) in accordance with state and federal regulations.
  • Coach, mentor, and support new nursing team members and nursing students as they grow in their careers.
  • Maintain a safe work environment by following all safety practices and procedures to protect yourself, your patients, and your colleagues.

Minimum Qualifications and Requirements

  1. Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).
  2. Three to four years of recent general medical/surgical or skilled/acute care nursing experience.

Expected Hours of Work

Sunday 8:00 a.m. - 8:00 p.m., Saturday 8:00 a.m. - 8:00 p.m,, Monday 8:00 a.m. - 4:30 p.m.

The team member will be scheduled to work based on program and Agency needs. Work assignments include holiday, weekend and/or on-call rotation.
