Job description

This is a part-time (up to four hours per week) contract position to provide homebound instruction as guaranteed by a student's medical 504 plan. The homebound instructor will:
  • serve as liaison between the student/parent and school insructional and administrative personnel (including serving as a member of the student 504 plan meetings)
  • support the student with completing and submitting assignments provided by instructional staff at the student's school
  • modify student assignments as outlined thorugh the student medical 504 plan
  • plan and provide instructional activities for the child through homebound instructional service delivery
  • keep documentation of all duties related to the contract (parent communication, instructional activities provided, timesheet, mileage
