Physical Therapist – Home Health (Contract)
Location: Delaware 19904
Contract Length: 13 Weeks
Pay: $2,159 – $2,421/week

Schedule:

  • Day Shift (8-hour shifts)

  • Monday–Friday

  • 40 hours guaranteed


Position Summary & Responsibilities (Concise)
  • Provide home health PT evaluations and treatments

  • Develop and implement individualized care plans

  • Document visits in HCHB (Homecare Homebase)

  • Monitor patient progress and adjust treatment plans

  • Educate patients/caregivers and coordinate care


Requirements

Education & Experience:

  • Graduate of accredited Physical Therapy program: Required

  • Home Health experience: Required

  • HCHB (Homecare Homebase) experience: Required

Licensure & Certifications:

  • Active Physical Therapist License (state of practice): Required

  • BLS/CPR certification: Preferred


Additional Notes
  • Strong documentation and time management skills required

  • Must be comfortable working independently in home settings
