Job Summary:
As an Infusion RN Case Manager with Allina Health Home Care, you'll build meaningful relationships with patients while coordinating and delivering specialized infusion therapy in the home setting. Key Position Details:
  • Casual (0.0 FTE)
  • Variable hours day/evening/night shifts
  • Occasional weekends and holiday availability
  • Some day-time and weekday availability is highly preferred
  • On-call may be required based on department needs
  • Travel will be required to patient homes for infusion education and administration. Travel will be required to office site intermittently (Currently in Mercy Hospital) and travel required for all training.
  • Eligible for mileage reimbursement; eligible after first 30 miles of travel, in between any stops and on return home, if greater than 30 miles from destination
Job Description:
Principle Responsibilities
  • Vascular Access and Medication Administration.
    • Maintains vascular access devices in accordance with current best practice guidelines, including maintenance of current vascular access devices or for the insertion of appropriate peripheral vascular access devices.
    • Educate patients and caregivers to appropriately infuse home intravenous or subcutaneous medications in the home setting.
    • Responsible for safe administration and monitoring of patient for intravenous medications required to be administered by nurse per departmental policy (i.e. biologics).
  • Home Environment and Care Coordination.
    • Assesses home environments for safe medication administration.
    • Ensures accurate documentation and appropriate plans per accreditation standards.
    • Obtains venipuncture laboratory specimens in accordance with best practice.
    • Coordination, and collaboration of care for patients to ensure care is provided in a safe, effective fashion.
    • Incorporates evidence-based nursing practice taking into consideration current statutes, rules and regulations when developing the plan of care.
  • Patient Outcomes and Quality Improvement.
    • Collaborates with patients and families to set realistic, measurable outcomes.
    • Participates in quality improvement strategies and to improve care quality and efficiency.
    • Implements interventions in a safe, timely, appropriate manner and escalates the patient to higher level of care if indicated.
  • Professional Practice Evaluation.
    • Evaluates one's own nursing practice in relation to professional practice standards and regulatory guidelines.
    • Engages in self-evaluation of practice on a regular basis, identifying strengths and goals for professional development.
  • Other duties as assigned.
Required Qualifications
  • Associate's or Vocational degree in Nursing
  • 2 years RN experience
Preferred Qualifications
  • Bachelor's degree-Health care related field (RN, PT, OT, etc)
  • Case management experience
Licenses/Certifications
  • Licensed Registered Nurse - MN Board of Nursing required if working in state of MN upon hire / WI Department of Safety & Professional Services required if working in the state of WI upon hire
  • Must meet at least ONE of the requirements below
    • Current BLS certification from the American Heart Association
    • Current BLS certification from the American Red Cross
    • Allina in-house BLS training (within 30 days of hire)
  • Certified Registered Nurse Infusion (CRNI) preferred
  • Vascular Access Board Certified (VA-BC) preferred
  • Valid Driver's License required upon hire - MN and WI residents must obtain a valid driver's license in their state of residence within 60 days of employment

Pay Range
Pay Range: $42.92 to $59.38 per hour
The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
