Home Health RN (Registered Nurse)

Empath Home Health, a member of Empath Health is seeking a compassionate Home Health RN (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes in Naples.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations: Home health visits in Naples.

Schedule: M-F 40 hours. Required weekend/on-call/holiday rotations.

What You'll Do

  • Perform comprehensive patient assessments, including OASIS assessments at required intervals
  • Develop, initiate, and revise individualized plans of care as patient needs evolve
  • Deliver skilled nursing services and treatments as outlined in the plan of care
  • Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
  • Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

  • Graduate of an approved nursing program with an active Florida RN license
  • Prefers BSN or MSN degree
  • Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
  • Prefers experience using Homecare Homebase (HCHB) EMR
  • Strong assessment, critical thinking, and patient education skills
  • Willingness to participate in weekend and holiday on-call rotations
