As a Home Health RN with Orion Home Health, you will play a vital role in providing high-quality, individualized care to patients in their homes, promoting independence, comfort, and well-being. Responsibilities:

  • Conduct initial patient assessments, develop personalized care plans, and provide skilled nursing care to patients in their homes

  • Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to ensure comprehensive care

  • Administer medications, perform wound care, and provide education to patients and their caregivers

  • Monitor patients' progress, report changes, and make recommendations for adjustments to care plans

  • Maintain accurate and detailed records of patient interactions, treatments, and progress
