We are a proud Joint Commission Accredited home health company.
ABOUT THE JOB:
We have an opening for a PER DIEM (field-based) California-Home Health Registered Nurse (RN) to provide skilled nursing visits to patients in Orange County and surrounding areas. This role requires strong clinical judgment, efficient documentation, and the ability to work independently in the field.
RESPONSIBILITIES:
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and routine skilled nursing visits.
  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Complete accurate and timely documentation (within 48 hours compliance)
  • Perform medication reconciliation at each visit and report changes.
  • Provide patient and family education based on diagnosis and care needs.
  • Collaborates with Care Coordinators, and interdisciplinary team.
  • Maintain compliance with Joint Commission, CDPH, and agency standards.
QUALIFICATIONS:
  • At least one year of nursing experience, including in a Home Health or similar environment is generally required (knowledge in OASIS documentation is a plus).
  • Strong clinical, organizational, and communication skills.
  • Ability to work independently and manage time efficiently.
  • Familiarity with EMR systems (MyUnity experience is a plus).
  • Being bilingual is a plus (Spanish, Vietnamese, Chinese).
CREDENTIALS REQUIRED:
  • Active California RN License.
  • Current CPR.
  • Driver's License.
  • Car Insurance.
  • Social Security Number.
  • Current Physical Exam and TB Test.
  • Immunization Records (Flu, Hep B, TDAP, MMR and Varicella)
