Position Overview

The Home Based Therapist provides community-based mental health services to individuals and families experiencing emotional or behavioral challenges. Services are delivered in homes, schools, and other community settings to help clients build stability and improve family functioning.

Home Based Therapists primarily serve families on the East side of Marion County, Hamilton County, Hancock County, and Shelby County.

What Success Looks Like in This Role

Within your first 90 days, you will:

  • Conduct bio-psychosocial assessments
  • Develop treatment plans for clients and families
  • Provide individual and family therapy
  • Connect clients with community resources
  • Maintain accurate clinical documentation
  • Collaborate with the clinical team to support client progress

Core Responsibilities

Clinical Services

  • Conduct assessments and develop treatment plans
  • Provide individual and family therapy in community settings
  • Provide crisis intervention when needed

Care Coordination

  • Work with treatment teams and community partners
  • Connect clients with community resources and services
  • Support discharge planning and long-term stability

Documentation & Compliance

  • Maintain accurate clinical documentation
  • Follow Behavioral Health Services policies and procedures
  • Protect client confidentiality

Minimum Qualifications

  • Master’s degree in a mental health-related field
  • Valid Indiana clinical license (LCSW, LMHC, or LMFT)
  • Minimum two years of mental health treatment experience
  • Valid Indiana Driver’s License and reliable transportation
