What You'll Do

  • Complete comprehensive Start of Care (SOC) admissions throughout the Twin Cities Metro.
  • Perform detailed nursing assessments and complete accurate OASIS documentation.
  • Develop individualized plans of care that set patients up for long-term success.
  • Educate patients and families on diagnoses, medications, services, and expectations.
  • Coordinate seamlessly with physicians, case managers, therapists, and the interdisciplinary care team.
  • Ensure smooth, safe transitions from hospitals, skilled nursing facilities, and clinics into home health.

What We're Looking For

  • Active Minnesota Registered Nurse (RN) license.
  • Strong clinical assessment and critical thinking skills.
  • Home health and OASIS experience strongly preferred.
  • Exceptional communication and patient education abilities.
  • Ability to work independently and manage a field-based schedule.
  • Reliable transportation.
