What You'll Do

  • Complete comprehensive Start of Care (SOC) admissions throughout the Twin Cities Metro.
  • Perform detailed nursing assessments and complete OASIS documentation.
  • Develop individualized plans of care based on each patient's needs and goals.
  • Educate patients and caregivers on medications, diagnoses, and home health services.
  • Coordinate with physicians, therapists, case managers, and the interdisciplinary care team.
  • Help ensure safe, successful transitions from hospital to home.

What We're Looking For

  • Active Minnesota Registered Nurse (RN) license.
  • Strong assessment, clinical reasoning, and documentation skills.
  • Home health and OASIS experience preferred.
  • Excellent communication and organizational abilities.
  • Ability to work independently in a field-based environment.
  • Reliable transportation.
