About the Job
Geriatrics Advanced Practice Provider - MedStar House Call Program - Baltimore
We are looking for an Advanced Practice Provider (NP or PA) to join our team in Geriatrics and Senior Services at MedStar Health. This position offers an opportunity to practice home-based primary care with a superb interdisciplinary team that serves underserved and high-need elders in Baltimore MD operating out of MedStar Good Samaritan Hospital (MGSH).
Responsibilities: The main duty is to provide home-based primary care (house calls) for a complex elderly population. Our mission is to provide skilled and compassionate care for elders with serious illness at home and across the continuum. The standard schedule is Monday-Friday, with occasional need for phone on-call coverage. You can be part of a motivated and skilled team of geriatricians, APP, social worker, RN, and office staff who provide compassionate care for underserved elders in D.C.
