Description

American Traveler is hiring an experienced RN - Home Care Coordinator (HCC) for a hospital-based discharge planning role requiring 2+ years of home care or discharge planning experience and an MD or Compact RN license.

Details
  • On-site position located at a major academic hospital main campus
  • Homecare Coordinator/Hospital Nurse Discharge Planner role within the Home Care Group
  • Primary schedule is Monday–Friday, 8:30 AM–5:00 PM
  • Weekend availability (Saturday and Sunday, 8:30 AM–4:00 PM) may be required on-site
  • No holiday or on-call requirements
  • Epic EMR used in this position
Requirements
  • Active MD or Compact RN license required
  • Current BLS certification required
  • Minimum 2 years of current home care, discharge planning, or case management experience required
  • Experience at a comparable large facility (500+ bed count) required
  • Epic EMR experience required
  • Thorough understanding of home care services, products, eligibility requirements, and community resources
  • SSN and DOB required for consideration
  • One professional reference required for consideration
Additional Information
  • Acts as a discharge planning resource for hospital staff involved in home care planning and facilitates continuity of care for patients transitioning to home-based services
  • Verifies patients have necessary durable medical equipment (e.g., walkers, commodes) and sets up home care services as needed
  • Advises patients and families on appropriate community support organizations
  • Provides patient and family education as part of the care transition process
  • Reports to the HCC/Discharge Planning Manager and serves as the on-site home-based care expert
