Family Support Specialist

Poughkeepsie, NY

Description

The Family Support Specialist provides intensive, home- and community-based clinical and case management services to children and families involved in the child welfare system. Under the supervision of the Program Director, the Specialist works as part of an interdisciplinary treatment team to prevent out-of-home placement and support family stability. Services are delivered using evidence-based practices in alignment with agency mission, contractual obligations, and Office of Children and Family Services (OCFS) requirements.

The Intensive Home-Based Services program is designed to promote child safety, permanency, and well-being, with a demonstrated history of maintaining the majority of enrolled children safely in their homes.

Job Responsibilities:

  • Provide direct, evidence-based clinical services to children and families in home and community settings, including clinical assessments, individual and family therapy, treatment planning, progress monitoring, and referrals.
  • Deliver case management and supportive services under professional supervision, including coordination with internal and external service providers.
  • Maintain accurate, timely, and compliant client records in accordance with agency policies, OCFS regulations, and funding requirements.
  • Prepare and submit required internal and external reports, including case progress reports to local Departments of Social Services.
  • Participate as an active member of the interdisciplinary treatment team to ensure coordinated and integrated service delivery.
  • Facilitate Family Team Conferences and support family engagement in service planning.
  • Provide aftercare services, including direct support and linkage to ongoing community resources.
  • Serve as liaison to the Department of Community and Family Services and other relevant stakeholders for assigned cases.
  • Provide crisis intervention services as needed, including participation in an on-call rotation.
  • Operate an agency vehicle in accordance with agency policy.

Requirements

You must have:

· Master's degree in Social Work or a related human services field (e.g., counseling, psychology).

· Minimum of one (1) year of relevant child welfare experience.

· Case management experience working with children and adolescents required.

· Valid New York State Driver's License with a driving record acceptable to the agency.

· Ability to travel to client homes and community locations.

· Ability to climb and descend stairs in environments that may not be compliant with the Americans with Disabilities Act (ADA).

You should have:

· Experience with home-based services and crisis intervention preferred.

· Functional Family Therapy (FFT) certification preferred; candidates may obtain certification through the agency within the first year of employment.
