Job description

Mulberry Health is a LOCAL clinician-owned mobile medical group providing in-home primary care, transitional care, and palliative care to patients across Arizona.

We care for seniors in private homes, assisted living communities, memory care communities, and group homes, bringing compassionate, high-quality healthcare directly to where patients are most comfortable.

Unlike traditional primary care practices, our providers have the opportunity to spend meaningful time with patients while building long-term relationships with families and caregivers. Our physician-led model ensures patients are seen quickly, efficiently, and with continuity of care.

As we continue to grow across Arizona, we are seeking a dedicated Nurse Practitioner to join our team of more than 20 providers delivering home-based primary care throughout the Phoenix Metropolitan Area.

Job Description

We are seeking a Primary Care Nurse Practitioner (NP) to provide in-home visits for patients with chronic conditions, serious illness, post-acute care needs, and general primary care requirements.

This is an excellent opportunity for a provider looking to practice independently while receiving strong clinical and operational support from an experienced leadership team.

As a provider with Mulberry Health, you will serve as the patient's primary care provider, managing their healthcare needs holistically while building lasting relationships with patients, caregivers, and families.

Multiple territories are available throughout the Phoenix Metropolitan Area, with assignments based on organizational needs and provider location.

Responsibilities

  • Conduct in-home visits within your assigned service territory
  • Provide comprehensive primary care services
  • Diagnose and manage acute and chronic medical conditions
  • Perform annual wellness visits and preventive care assessments
  • Coordinate care with specialists, hospitals, caregivers, hospice teams (when appropriate), and family members
  • Document visits using eClinicalWorks (eCW)
  • Educate patients and caregivers on health maintenance, disease prevention, and symptom management
  • Participate in interdisciplinary care planning
  • Support care transitions and medication management
  • Deliver exceptional patient-centered care to seniors and medically complex patients

Qualifications

Required

  • Certified Nurse Practitioner (FNP-C, AGNP-C, or equivalent)
  • Active Arizona Nurse Practitioner License
  • Active DEA Registration or ability to obtain
  • Valid Arizona Driver License
  • Reliable transportation
  • Strong communication and clinical documentation skills
  • Ability to work independently and manage a flexible schedule

  • Primary Care experience
  • Home-Based Primary Care experience
  • Geriatric experience
  • Home Health experience
  • Assisted Living or Group Home experience
  • Palliative Care experience
  • Experience caring for Medicare populations
  • Knowledge of value-based care models

Compensation & Benefits

$120,000 - $180,000+ Per Year
