Now Hiring: Registered Nurse (RN) – One Heart Program | Home Health

Preferred Care Home Health is seeking an experienced Registered Nurse (RN) to join our specialized One Heart Program, providing skilled cardiac and cardiopulmonary care to patients in the comfort of their homes.

This role is ideal for an RN who enjoys working independently, providing patient education, coordinating care, and making a meaningful impact on patient outcomes. You’ll work closely with physicians, our One Heart APRN, therapists, patients, and caregivers to support recovery and help reduce hospital readmissions.

What You'll Do
  • Provide skilled nursing care to cardiac and cardiopulmonary patients.
  • Complete comprehensive assessments and individualized care plans.
  • Monitor patient progress and communicate changes in condition.
  • Collaborate with physicians, APRN, therapists, and the interdisciplinary team.
  • Educate patients and caregivers on medications, disease management, safety, and recovery.
  • Assist with Remote Patient Monitoring (RPM) setup and education.
  • Administer skilled nursing treatments, including IV Lasix for eligible CHF patients.
  • Maintain accurate and timely clinical documentation.
Qualifications
  • Active Florida RN license.
  • Graduate of an accredited nursing program.
  • Minimum 1 year of medical-surgical nursing experience.
  • Cardiac, telemetry, cardiovascular, cardiothoracic, or home health experience preferred.
  • Strong assessment, communication, and care coordination skills.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and auto insurance.
