Job description

Home Based Primary Care Outpatient Staff Registered Nurse

Duties and responsibilities of the HOME BASED PRIMARY CARE OUTPATIENT STAFF REGISTERED NURSE include, but are not limited to:

  • Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal outpatient care.
  • This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program.
  • Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.
  • This nurse plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care.
  • Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care.
  • Makes home visits to Veterans in the program.
  • The focus is on individual growth and development in HBPC nursing practice with increased levels of self direction.
  • Seeks guidance on how to improve care provided to patients and families.
  • Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient.
  • The nurse brings HBPC principles and theory with limited application of critical thinking skills.
  • Responsible for participating within unit based and/or organizational level Shared Governance councils, utilizing clinical expertise, evidence-based practice, and knowledge to assist with finding solutions to problems or issues to improve the quality care provided to our veterans.

Work Schedule: Monday - Friday 7:30am - 4:00pm/ 8:00am - 4:30pm no weekends/ no holidays

Relocation/Recruitment Incentives: Authorized to highly qualified and eligible new federal appointees or former federal employees with at least a 90-day break in service.
