Overview
Travel Registered Nurse – Home Health in Kaneohe, Hawaii. Start date: 08/10/2026. Assignment duration: 13 weeks. Schedule: 5x8 days. Guaranteed hours: 40 per week. Weekly pay: $2,261–$2,356. Setting: Home Healthcare.

Responsibilities
- Lead coordination of patient care within the home health team, including case management program development, implementation, revision, and reporting; serve as liaison among patient, family, nurse, physicians, and multidisciplinary team to optimize outcomes.
- Act as a clinical consultant to the care team on specific patient items; oversee and coordinate the interdisciplinary care plan, ensuring timeliness and appropriateness of care.
- Monitor the patient’s clinical course, including diagnostic testing, treatments, procedures, and other care elements, to ensure alignment with the continuing care plan and covered services.
- Discuss with physicians the appropriateness of resource utilization, treatment plans, estimated length of stay, and discharge planning; focus on complex patients, ED utilizers, chronic pain and substance use disorders, and homeless patients.
- Collaborate with acute care case managers to ensure ongoing care appropriateness; coordinate transfers to tertiary centers and ensure transfer of patient information for continuity of care.
- Provide oversight and collect data required for regulatory and accreditation compliance; manage focus studies of frequent ED visits to inform care improvements.
Requirements
- Hawaii licensed Registered Nurse with an active and clear license.
- 1+ year Medicare-certified home health agency experience (required).
- Active and clear driver’s license and car insurance; reliable personal transportation.
- Proficiency with Homecare Home Base and OASIS (required documentation).
- COVID-19 and influenza vaccines on file (documentation required; no declinations accepted).
- Bachelor’s degree preferred.
- Case management experience preferred.
- Ability to work in a home health/home-based care setting; strong coordination and communication skills with multidisciplinary teams.
