• Conduct comprehensive assessments to evaluate and identify the needs of patients, ensuring that care plans are tailored to their unique circumstances.
• Maintain clear and collaborative communication with supervisors, direct care staff, family members, and other stakeholders to discuss patient progress and make necessary adjustments to treatment plans.
• Schedule and manage appointments, home visits, and observational assessments to effectively monitor patient advancement and implement feedback into care strategies.
• Coordinate and conduct in-home observations, chart reviews, and participant interviews as part of a holistic assessment process.
• Ensure that treatment program plans are effectively executed and properly documented, guaranteeing compliance with organizational standards.
• Participate actively in Interdisciplinary Team Meetings, working collaboratively to devise strategies and recommendations that enhance patient care.
• Complete progress notes on a monthly and quarterly basis to document developments and inform future treatment approaches.
• Organize and conduct training sessions to ensure that all staff members are proficient in program protocols and documentation practices.
• Perform follow-up interviews and home visits to provide continued support and adjustment of care as necessary.
• Maintain accurate and up-to-date records, ensuring accessibility of information to relevant parties and compliance with legal requirements.
• Act as a liaison within the mental health community to foster collaboration and support for program development initiatives.
• Contribute to the creation and enhancement of training materials designed to improve programming for individuals.

You will engage with a diverse clientele, including both adults and youth, across various backgrounds and challenges. Specializing in areas such as family therapy, couples therapy, trauma recovery, and substance misuse, your role will involve providing guidance and support tailored to the specific needs of your patients. Your expertise in these areas will greatly enhance the quality of care offered to our community.

• Education: A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• Licensure: Must possess active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.
• Experience: A minimum of two years of counseling experience is required, preferably in a clinical setting.
• Expertise: Experience in specialized areas such as family therapy, trauma, or substance misuse is highly preferred.
• Skills: Strong interpersonal skills, a compassionate demeanor, and excellent communication abilities are essential for establishing rapport and trust with clients.
