Job description

POSITION SUMMARY:
The Home Based Therapist position provides a wide range of services to children and their families when the children are at risk of out-of-home placement or hospitalization. Most clinical services will be provided in the children's homes or in the community to stabilize and strengthen child/family functioning and/or to prevent or forestall placement. Treatment is time limited. Staff in this position function with a great deal of professional autonomy under the supervision of the program supervisor.
EDUCATION AND / OR EXPERIENCE:
  • Master's degree in psychology, social work or another related human services field required.
  • Current licensure/certification in the State of Michigan required.
  • At least one year of experience in the examination, evaluation, and treatment of minors is required.
  • At least one year experience with multiple major human service delivery systems (FIA, public health, education, etc.) preferred.

DUTIES AND RESPONSIBILITIES:
  • Maintain clinical charts in an accurate and timely manner to include, but not limited to assessments, treatment plans, and all other MDCH chart requirements.
  • Maintain a caseload size and weekly productivity expectations as determined by supervisor.
  • Provide individual, group, and family treatment for children with mental health and co-occurring disorders (developmental disabilities and/or substance abuse).
  • Provide education, consultation, and case management services.
  • Facilitate client access to other service delivery systems, e.g., D.S.S. special education, etc.
  • Assure the coordination and integration of foregoing services with those provided by Starfish Family Services.
  • Make intra-agency referrals for evaluations, consultation, and other services, e.g., parent education.
  • Maintain cooperative working relationships with educational, legal, health, social welfare, and mental health systems.
  • Secure relevant reports from and provides reports to the foregoing system.
  • Participate in the planning, development, and evaluation of agency programs.
  • Attend mandatory agency meetings, trainings and supervisions.
  • Participate in professional development activities, including in-service training, peer review, external workshops, and seminars, etc.
  • May participate in special projects, e.g., research, community education, etc.
  • Cooperate in any investigation related to personnel, licensing, accreditation or other circumstances.
  • Work cooperatively as a member of a team with program staff and community resources.
  • Accept and emulate the mission of SFS.
  • Must maintain professional and ethical standards at all times.
  • Assist in finding and directing clients in case of an emergency such as a fire or tornado.
  • Adhere to all Lifespan policies and procedures as it relates to Documentation, Training requirements, CMH guidelines, and Confidentiality.

COMPETENCIES AND ABILITIES:
  • Knowledge of theory and practice of individual, group, and family systems treatment.
  • Competence in the delivery of crisis intervention services, and in brief and time-limited therapy.
  • Development and Implementation of Individualized Plan of Service utilizing Person Centered Planning.
  • Competency in performing, or willingness to learn, a wide range of treatment modalities that are individualized and person centered.

LICENSING AND OTHER REQUIREMENTS:
  • Must possess a current valid State of Michigan chauffeur's driver's license and automobile insurance that is insurable. Must meet SFS and our insurance carrier guidelines for the transportation of children/ clients.
