Overview
As a Registered Nurse-Admission Liason for Infusion, you would be tasked to Assess patients individually for service needs once patient choice has been elicited from appropriate hospital staff and/or referral is made to MH from other referral sources. The Admission Liaison is accountable for either home care or infusion needs assessments, as assigned. Explains MH program services to patient/family and functions as a liaison between referral source and MHS intake, BJC pharmacy, and/or Home Health intake, as needed. Proactively educates physician and family, and acts as a resource for MH System affiliates, referral hospitals and nursing staff regarding MH infusion, home health services, Medicare coverage, and/or regulatory limitations for admission, etc. Provides bedside teaching to patients and caregivers.
Qualifications
Education:
  • Graduate of an accredited College of Nursing, with an Associate's degree (ASN) required
  • BSN preferred

Licensure/Certification/Registry:
  • Licensed RN in the State of Illinois required
  • Current BLS/CPR certification required within 45 days of hire
  • Infusion Nurse Certification, preferred for those assigned to Infusion Services

Experience:
  • Minimum of three (3) years relevant nursing experience, required
  • One (1) year experience in home health/hospice or infusion services, preferred

Other Knowledge/Skills/Abilities:
  • Ability to perform health assessments, required.
  • Familiarity with Medicare, IPA and other insurance, preferred.
  • Proactive, positive, problem-solving approach, required.
  • Excellent customer service and verbal communication skills, required.
  • Demonstrated ability to be a team player, required.
  • Basic computer skills, required.
  • Must possess a valid IL driver license and must be deemed an acceptable driver in accordance with the MHS Fleet Safety Policy (five (5) year MVR will be required)

Responsibilities
  1. Establishes a close team relationship with referral sources, MHS case managers and discharge planning, as well as discharge planners of other facilities and/or hospital departments. May need to work closely with BJC Pharmacy, as needed.

  1. Explains program philosophy, care issues and practices, and regulatory restrictions to referring hospital, physicians and others in order for optimal care to be achieved.

  1. Serves as a resource to MH personnel, other agencies and hospital personnel and the community.

  1. Upon referral, contacts and interviews patients and families to assess and evaluate patient's social, financial and environmental situations; and to explain infusion and/or home health programs, benefit coverage and eligibility for infusion and/or home health services. Evaluates patients to determine eligibility.

  1. Upon referral, works with the case manager and/or infusion pharmacy to develop a discharge plan for the hospitalized patient based on patient/family needs.

  1. Works with patient, family, physicians and staff to promote safe, appropriate and timely hospital discharge. Coordinates physician/family referrals and start of care. Works with the ordering physician and pharmacist, to develop an appropriate initial plan of care.

  1. Works with Insurance Liaison and/or infusion pharmacy to determine coverage issues. Informs family of coverage for service and medications

  1. Provides bedside teaches as needed to help facilitate timely discharge from the hospital as required.

  1. Communicates/coordinates referrals with MHS Intake, Coordinated Home Care Services and/or Infusion Pharmacy.

  1. Participates in home health team functions and quality safety improvement projects as requested.

  1. Reassesses needs of currently hospitalized MHS Home Health patients as required.

  1. Works weekends/holidays at intake/ scheduling desk as requested.

  1. Maintains statistics as requested.

  1. Follows agency policies and procedures.

  1. Performs other related work as required or requested.
