Job description

Clinical Pharmacist - Home Infusion

This is an opportunity to play a defining role in advancing high-acuity care beyond the walls of the hospital. As UVA Health continues to expand its specialty home infusion capabilities, this position places you at the center of delivering complex, life-enhancing therapies to patients in a highly coordinated and clinically rigorous model. As a frontline clinical pharmacist in a rapidly scaling specialty program, you will manage complex patients, guide therapy outcomes, and help shape the future of home-based infusion care. This is a role built for pharmacists who want meaningful patient impact, clinical depth, and the chance to contribute to an emerging model of care delivery.

The Role In this role, you will serve as a clinical leader within a high-growth specialty home infusion program, managing a dedicated panel of complex patients while collaborating across a multidisciplinary care team. This is a non-dispensing, clinically focused position that emphasizes patient outcomes, coordination, and program development. On a typical day, you will:

  • Manage a panel of approximately 50 to 60 specialty infusion patients, supporting them from therapy initiation through stabilization
  • Provide ongoing clinical monitoring, evaluating therapy effectiveness, side effects, and adherence for high-cost, high-complexity medications
  • Conduct proactive patient outreach and education through a remote, telephonic care model, building trusted longitudinal relationships
  • Serve as a central liaison between patients, providers, and pharmacy teams, ensuring seamless coordination of care
  • Collaborate closely with physicians, clinic pharmacists, and internal teams to adjust therapies and resolve complex clinical scenarios
  • Oversee refill coordination and therapy continuity, ensuring safe and timely medication access
  • Participate in weekly process improvement and program development initiatives, contributing to workflow design and policy creation
  • Support operational planning such as high-value medication ordering and forecasting, ensuring alignment with patient needs

MINIMUM REQUIREMENTS: Education: Bachelor's in Pharmacy or Pharm.D. from an ACPE accredited College of Pharmacy required. Experience: No experience required. PGY-1 residency or equivalent clinical experience preferred. Licensure: Virginia State Board of Pharmacy License or within 60 days of hire. American Heart Association (AHA) Health Care Provider BLS certification required. PHYSICAL DEMANDS: Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull less than 20lbs. May be exposed to fumes, chemicals, blood/body fluids and infectious disease. The pay range for this role is $64.37 - $87.33 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
