Position Overview

We are seeking a dedicated and passionate Home-Based Physical Therapist to join our team at Lakeland Care Solution in Avenel, New Jersey. This full-time role will involve providing high-quality physical rehabilitation services to patients in their home environments. You will work on improving patient outcomes through tailored treatment plans and evidence-based practices, with a salary range of USD 100,000 to 120,000 annually.

Work Environment

Your primary work setting will be patients' homes, where you'll provide personalized, hands-on care tailored to individual needs. Here, you will utilize your assessment skills and therapeutic techniques in various home settings, including those of elderly patients and individuals recovering from orthopedic surgeries. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions, functionality, and mobility, developing individualized treatment plans based on physician referrals.
• Implement diverse therapeutic interventions, addressing a variety of conditions including orthopedic, pediatric, neurologic, and sports-related injuries.
• Engage with patients to provide ongoing education, encouraging their active participation in the rehabilitation process.
• Document all patient interactions and treatment outcomes using our electronic health record systems to ensure clear communication and continuity of care.
• Foster strong relationships with patients and their families to enhance their trust and motivation, which are crucial for recovery.
• Meet and exceed outlined business goals and utilization metrics through dedicated patient care and community outreach.

Patient Population / Client Focus

Your client focus will include a wide range of patients, from children with developmental challenges to adults recovering from surgeries such as knee or hip replacements. You will develop specialized programs that cater to their unique needs, helping them regain independence and improve their quality of life.

Qualifications

• Minimum of a Bachelor's degree in Physical Therapy, with a preference for candidates holding a Doctorate of Physical Therapy (DPT).
• Current state license in Physical Therapy in New Jersey.
• Certification in Basic Life Support (BLS) from the American Heart Association.
• Proficient in written and verbal communication skills, enabling effective patient education and teamwork.
• Demonstrable proficiency in computer skills and electronic documentation.
• An empathetic nature with a genuine desire to connect with patients on a personal level to enhance their rehabilitation experience.

Preferred Experience

• At least two years of clinical experience in various therapy settings, including home health care.
• Experience with evidence-based practice methodologies and a commitment to ongoing professional development.
• A background in community engagement or outreach programs is highly desirable.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 per year, complemented by a robust benefits package that promotes employee wellbeing.
