Job description

Field RN Home Health Visits in Sebastian / Barefoot Bay
The RN will work to provide the highest quality of care to patients utilizing the Plan of Care provided to ensure patient's needs are met in relation to safety, well-being and satisfaction. WellSky/Kinnser, Oasis and Home Health experience is a plus.
JOB RESPONSIBILITIES
  • RN will be supervised by an DON
  • Provides safe and effective care for the client based on individualized Plan of Care.
  • Provides prompt communication of changes in patient medical status/emergencies to Team resulting in effective management of cases.
  • Provides safe and effective nursing for the patient, based on the nursing scope of practice and individualized Plan of Care in conjunction with Nurse Supervisor/Manager of Clinical Practice. May include, but not limited to, medication administration, wound care, infusion, transferring & positioning patient, catheter care, effective communication with the client, family, and Team Case Manager
  • Demonstrate empathy, caring and self-confidence; ability to pay attention to details resulting in thorough follow through; anticipate problems in advance and knows whether to diffuse or escalate; prioritize and demonstrate a sense of urgency.

REQUIRED EXPERIENCE & EDUCATION
  • Current Nursing, state of Florida
  • Possess wound care skills
  • Excellent communication skills; team player; interpersonal skills; good collaboration skills; customer service skills; ability to use the electronic health record for documentation and communication regarding client care.
  • 1 year of Home Health experience preferred
