Nurse Practitioner (FNP/AGNP) – Telehealth & Complex Care

Location: Delaware / Remote Telehealth
Job Type: Full-Time | 40 hours per week
Salary: $135,000/year



Position Overview

We are seeking an experienced Nurse Practitioner (NP) to join an innovative, growing healthcare organization delivering technology-enabled, value-based care to medically complex patients throughout Delaware.


This is not a traditional high-volume clinic position. The NP will provide primarily virtual care and proactive clinical management to adults with complex medical and functional needs, helping patients remain safely at home while reducing preventable emergency department visits and hospitalizations.


You'll work as part of an interdisciplinary team that includes physicians, Nurse Practitioners, Registered Nurses, Licensed Clinical Social Workers, therapists, and other healthcare professionals.


Patient Population

The NP may manage Medicaid and Dual-Eligible patients with:

  • Multiple chronic medical conditions
  • Physical disabilities
  • Traumatic brain injuries (TBI)
  • Intellectual and developmental disabilities (IDD)
  • Functional limitations
  • Complex geriatric conditions
  • Significant care-coordination needs


Key Responsibilities

  • Provide clinical care and ongoing patient management through telehealth and remote-care technologies
  • Assess and manage medically complex adult and geriatric patients
  • Develop and execute individualized care plans
  • Collaborate with physicians and interdisciplinary clinical teams
  • Coordinate care with patients, families, caregivers, PCPs, specialists, and health-plan partners
  • Identify changes in patient condition and intervene early to prevent avoidable ED visits and hospitalizations
  • Support patients through hospital admissions, discharges, and transitions back into the home
  • Participate in advance-care planning and goals-of-care discussions
  • Participate in shared clinical triage and patient-support activities
  • Maintain timely and accurate clinical documentation
  • Support HCC coding and risk-adjustment initiatives
  • Assist with closure of quality and care gaps
  • Support STAR, HEDIS, and other quality initiatives
  • Contribute to clinical best practices and the continued development of the organization's complex-care model


Required Qualifications

  • Master's degree or higher
  • Current Nurse Practitioner certification
  • FNP, NP, or Gerontological/Adult-Gerontology NP certification appropriate to the patient population
  • Active Delaware NP license OR active NP license in a bordering state with willingness to obtain Delaware licensure
  • At least 1 year of telemedicine, virtual care, and/or care-management experience
  • Ability to practice with appropriate prescriptive authority
  • Experience managing adult and/or geriatric patients
  • Strong clinical judgment and ability to work independently in a remote-care environment
  • Strong communication, documentation, and interdisciplinary collaboration skills


Delaware licensure support: Qualified candidates who do not currently hold a Delaware license may be considered. The organization is willing to support and cover the cost of obtaining Delaware licensure for the selected candidate.

This position is particularly well suited for an NP coming from value-based care, population health, managed Medicaid, home-based primary care, geriatrics, telehealth, complex care management, SNF, home health, rehabilitation, or emergency/triage medicine.

The successful candidate will be comfortable practicing independently while remaining highly engaged with an interdisciplinary team.
