As a Licensed Practical Nurse LPN, you'll contribute to our success in the following ways:

  • Deliver in-home nursing care aligned with patient care plans

  • Review and follow Plan of Care prior to each visit

  • Communicate patient updates, concerns, and changes in condition

  • Assist with medication management and documentation

  • Educate patients and families on self-care techniques

  • Identify safety concerns and help prevent hospitalizations

  • Collaborate with RN Case Managers, physicians, and care team

What is required?

  • Current LPN/LVN license, in the state of Missouri

  • Graduate of an accredited nursing program

  • At least 1 year of experience in acute care ( home care, home health or hospice preferred)

  • Strong communication and interpersonal skills

  • Ability to manage time effectively and stay organized in the field

  • Knowledge of healthcare regulations and patient care standards

  • Compassionate, patient-first mindset

  • Reliable personal transportation (travel required ~50%)

  • Ability to meet health screening and compliance requirements

You will report to the Clinical Manager or Branch Director for Personal Care Services.
