What You'll Do

As a Home Health RN, you'll play a vital role in helping patients recover, manage chronic illnesses, and remain safely at home by:

  • Performing comprehensive nursing assessments in patients' homes.

  • Developing, implementing, and evaluating individualized plans of care.

  • Providing skilled nursing care, education, and disease management.

  • Monitoring patient progress and identifying changes in condition.

  • Collaborating with physicians, therapists, caregivers, and interdisciplinary team members to coordinate care.

  • Educating patients and families on medications, treatments, and disease management.

  • Accurately documenting visits and maintaining timely clinical records.

  • Ensuring compliance with all clinical standards, agency policies, and HIPAA regulations.

  • Advocating for patients while promoting the highest quality of care.

Qualifications
  • Current, unrestricted Massachusetts Registered Nurse (RN) license.

  • Minimum of one year of nursing experience in a clinical setting.

  • Home Health experience preferred, but not required.

  • Experience with OASIS documentation is preferred.

  • Experience with Homecare Homebase is a plus.

  • Strong assessment, communication, and organizational skills.

  • Associate's Degree in Nursing required; Bachelor's Degree preferred.

  • Valid driver's license, reliable transportation, and current auto insurance.

  • Ability to travel throughout the Newton service area.

  • Ability to safely lift, push, pull, and assist with up to 50 pounds as needed.
