Job description

Home Based Primary Care Outpatient Staff Registered Nurse (RN)

The Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex HBPC care. Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Partners with the patient, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum, including the Veteran's goals of care. Delivers fundamental knowledge-based care to assigned clients while developing technical skills. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Catchment areas from Vero Beach to Boca Raton. The duties include but are not limited too:

  • Make home visits to Veterans in the program and assess, plan, implement, and evaluate care based on age specific components.
  • Assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
  • Influence care outcomes by collaborating with members of the interdisciplinary team.
  • Support colleagues and other nurses through HBPC knowledge sharing to provide safe, quality nursing care, and foster a safe and supportive environment conducive to the professional development of health care professionals.
  • Evaluate outpatient outcomes of evidence-based decisions and practice changes for HBPC individuals, groups, and populations.
  • Responsible for the documented HBPC outcomes at the program or service level.
  • Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
