Job Description

CTVHCS is in the process of making a cultural change, shifting from a disease-focused approach to a health and wellbeing approach, in order to implement the Whole Health System of Care. This is a VA-wide initiative to provide a comprehensive and systematic approach to improve healthcare delivery at all stages of Veteran care. This entails moving from an episodic, "siloed" approach to an interdisciplinary, holistic approach, emphasizing self-care in the larger context of well-being. All staff in this facility will be part of this process, shifting the focus from the question, "What's the matter with you?" to the question, "What matters to you, and how can we help you live your best life?" This strategy also involves integrating a full range of conventional approaches with Complementary and Integrative Health (CIH) approaches in order to optimize lifelong health and well-being, as defined by the Veteran, rather than narrowly focusing on disease management.

Major duties include, but are not limited to:

  • Provides comprehensive home-based nursing care, including assessment, triage, case management, medication and treatment coordination, and individualized care planning.
  • Conducts home visits to evaluate patient safety, functional and cognitive status, environmental risks, and age- and culturally-specific care needs.
  • Delivers evidence-based, technically competent care supporting health promotion, chronic disease management, geriatric care, primary care, and palliative care.
  • Collaborates with physicians, advanced practice providers, social workers, therapists, and mental health professionals to ensure fully integrated, patient-centered care.
  • Utilizes interventions designed to maximize health capabilities and reduce avoidable urgent care and emergency department visits.
  • Participates in quality improvement, performance monitoring, and data collection activities to strengthen HBPC program outcomes.
  • Ensures accurate, complete, and timely documentation in accordance with all clinical and regulatory requirements.
  • Educates Veterans, families, and caregivers on disease management, treatment plans, medication adherence, and available resources.
  • Demonstrates cultural competence and sensitivity to the developmental and age-related needs of diverse Veteran populations.
  • Maintains clinical competency through ongoing professional development and supports evidence-based practice within the HBPC program.

Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Licensure: 1 full and unrestricted license from any US State or territory
