This position is primarily based onsite, as you will be traveling to clients’ homes throughout Cypress, Florida. This home care setting allows you to work closely with patients in a familiar and comfortable environment, fostering strong therapeutic relationships. You will collaborate with a dedicated interdisciplinary team while having the autonomy to develop and execute individualized treatment plans aimed at achieving the best possible patient outcomes.

As a Home-Based Orthopedic Physical Therapist, your day-to-day responsibilities will include:

• Directing physical therapy treatment focusing on pain relief, muscle function improvement, and disability prevention, aligning with the established plan of care.
• Supervising Physical Therapy Assistants (PTAs) to ensure they implement therapy plans accurately and safely.
• Conducting comprehensive evaluations of clients’ functional abilities through diagnostic tests to tailor treatment strategies accordingly.
• Assessing each client’s home environment and recommending modifications to enhance their independence and ability to perform daily activities.
• Providing hands-on physical therapy services and educating clients and their families on exercise routines and therapeutic devices.
• Preparing thorough clinical documentation, including initial evaluations, progress notes, and discharge summaries, in alignment with compliance standards.
• Participating in team meetings and case conferences to collaboratively revise care plans based on patient progress.
• Maintaining open communication with clients, families, and other healthcare professionals to ensure cohesive care delivery.

Our clientele includes individuals recovering from orthopedic surgeries, accidents, or physical impairments that impact their mobility and daily activities. To be considered for this role, candidates must possess:

• A bachelor’s degree in physical therapy from an accredited institution.
• A current PT license in good standing in the state of Florida.
• Current CPR certification.
• A valid driver’s license along with reliable transportation and automobile insurance.
• A creative mindset with the ability to adapt therapy plans based on patient progress and challenges.

While not mandatory, the following experiences are preferred:

• At least one year of experience working as a Physical Therapist in acute care or home care settings.
• Experience working with orthopedic patients specifically, demonstrating knowledge of the latest rehabilitation techniques and modalities.
