Position Overview

Flex Care Chiropractic Center is seeking a dedicated and skilled Home-Based Orthopedic Physical Therapist to provide exceptional care to our clients in the Chesapeake, Virginia area. This full-time role focuses on delivering customized physical therapy solutions within the client’s home environment, enabling individuals to achieve optimal recovery and enhance their quality of life.

Work Environment

This position is primarily onsite, performed within the comfort of clients’ homes across the Chesapeake region. You will operate in diverse settings, adapting to the unique challenges each home environment presents, and utilizing advanced physical therapy tools and techniques. As a vital member of our healthcare team, you will work alongside other healthcare professionals to coordinate care and ensure our clients receive the utmost attention and support.

Key Responsibilities
  • Direct and oversee individualized physical therapy interventions aimed at preventing disabilities, alleviating pain, and enhancing or restoring optimal muscle function based on physician-prescribed treatment plans.
  • Supervise the work of physical therapy assistants, ensuring adherence to treatment protocols while providing mentorship and guidance in patient care methodologies.
  • Conduct thorough evaluations of each client’s functional abilities through diagnostic assessments, determining their physical capabilities and areas requiring focused intervention.
  • Assess clients’ home environments to identify modifications or enhancements that support rehabilitation and promote independence.
  • Deliver in-home therapy sessions that include manual therapeutic techniques, therapeutic exercises, and the application of physical agents such as traction and massage.
  • Evaluate, fit, and adjust prosthetic and orthotic devices while collaborating with the prescribing physician for modifications as necessary.
  • Contribute to the development and continuous improvement of comprehensive care plans through ongoing assessment and collaboration with interdisciplinary teams.
  • Document patient progress meticulously, prepare clinical records, and systematically record changes in client conditions to inform treatment adjustments.
  • Participate in team case conferences to discuss client progress and strategy, ensuring a coordinated approach to treatment.
  • Provide education to clients and families regarding physical therapy treatment plans, device use, and self-care practices, reinforcing the therapy goals.
  • Remain reachable to Physical Therapy Assistants during client treatment sessions to provide immediate support and guidance.
  • Meet all administrative duties, including timely documentation submissions in compliance with company policies.
Patient Population / Client Focus

Our client demographic primarily includes individuals recovering from orthopedic surgeries, managing chronic musculoskeletal issues, or seeking rehabilitation from injuries—those who benefit greatly from tailored home-based therapy. We emphasize a holistic approach, integrating family into the rehabilitation process to foster a supportive atmosphere that motivates clients toward recovery and independence.

Qualifications
  • Doctor of Physical Therapy (DPT) degree from an accredited institution.
  • A valid physical therapy license in Virginia, in good standing, with the ability to operate in compliance with the state's regulations.
  • At least one year of relevant experience as a Physical Therapist in acute care or home care settings.
  • Current CPR certification and a valid driver’s license with automobile insurance.
  • Demonstrated excellence in interpersonal skills, with the ability to communicate effectively with patients, families, and other healthcare professionals.
  • Self-motivated with a strong commitment to clinical excellence and continuous professional development.
Preferred Experience
  • Experience in home health care settings focusing on orthopedic rehabilitation is preferred.
  • Proficiency in federal, state, and governing agency regulations related to home healthcare services.
  • Strong presentation and documentation skills targeting patient progress and treatment effectiveness.
Compensation and Benefits

This rewarding position offers a competitive salary range of USD 90,000 to 100,000 annually, commensurate with experience.
