Job description

JOB SUMMARY

Position Title: Thrive to Five Home Visitor

Program: Education and Child Development Impact Team

Classification: Non-Exempt/Full-Time

Location: Columbia, PA

The Thrive to Five Home Visitor will oversee the planning, implementation and reporting of home visits, family engagement, resource connections, recruitment and community outreach.

Duties & Responsibilities:

Home visitation

  • Complete home visitation frequency in accordance with funders, as directed by supervisor. Recruit families and complete all necessary application and enrollment forms.

Developmental screenings and family assessments

  • Assess children through required developmental and health screenings, and performance measure assessments.
  • Monitor age-appropriate milestones and support healthy child development. Complete family assessments to support goal setting process.
  • Provide appropriate resource connections.

Data collection

  • Plan, implement and record all home visitation and required documentation according to funders and curriculum guidelines.
  • Maintain up to date, thorough and accurate home visiting records, including regular data entry and evaluations, in the appropriate database(s).
  • Document caregiver goals to support family well-being, monitor progress on goals regularly.
  • Document and track required developmental, mental and physical health screenings and documentation.

Community collaboration

  • Provide family support through community resources and socialization opportunities.
  • Make appropriate referrals to community resources and advocate on behalf of families when appropriate.
  • Work collaboratively to plan, implement, and encourage family attendance during family engagement events.
  • Use strong, interpersonal skills when interacting with families and local partners, and demonstrate flexibility when changes arise.
  • Facilitate communication and collaboration between community partners to improve kindergarten readiness and overall family wellness.
  • Participate in first 10 meetings and community partner meetings, as appropriate.
  • Participate in all required team meetings.
  • Develop an understanding of a commitment to the mission of cap, as well as the mission of community partners, as appropriate.

Professionalism

  • Work independently and have a strong sense of responsibility in carrying out the required job duties with a minimum amount of daily supervision.
  • Meet regularly with the supervisor for reflective supervision.
  • Identify professional development opportunities to continually increase knowledge of kindergarten readiness skills to share with families.
  • Demonstrate cap values during interactions with families, team members, supervisors, and community partners.

Maintain confidentiality regarding program families.

*Perform other duties as assigned, including occasional classroom coverage.

EDUCATION/EXPERIENCE REQUIREMENTS

Required:

  • CDA and 3 years’ experience working with families with young children.

Preferred:

  • Associate’s degree in social work, ECE, or related degree.
  • Bachelor’s degree in social work, ECE, or related degree.
