Job Summary/Objective:
Home-Based services is a community based therapeutic service within the youth services continuum.ย Home-Based clinicians provide intensive, therapeutic services to youth and families with severe emotional disturbances.
Essential Duties and Functions:
- Completes psychosocial assessments inclusive of a diagnosis, case conceptualization, level of care assessment, and recommendations for treatment.ย
- Engages in person-centered planning to develop goals and objectives.ย ย ย
- Utilizes evidenced based treatments within their scope of practice.ย
- Engages in risk assessment and completes safety planning with youth and families.ย
- Coordinates appropriate referrals to other youth-oriented programs such as Outpatient, Wraparound, and/or Youth Peer Support Services.ย
- Performs outreach prevention activities within the community.ย
- May conduct educational presentations.ย
- Maintains record compliance according to agency guidelines and standards as well as funding source regulations.ย ย ย
- Shall ensure compliance and participation with competency improvement activities including, but not limited to credentialing, continuing education (per licensure requirementsย or a minimumย of 24 hours per year of clinically based trainings), implementation of evidence-based practices, peer review attendance, and professional improvement practices.
Qualifications:
- Masterโs Degree in Psychology, Social Work or related field with Certification in the State of Michigan as a Licensed Master's Level Social Worker (LMSW) or Limited Licensed Psychologist (LLP) or Licensed Professional Counselor (LPC) preferred; Limited Licensed Master's Level Social Worker (LLMSW) or Limited Licensed Professional Counselor (LLPC) requires supervision by a fully licensed professional within their discipline.ย
- Two years of experience in a behavioral health care setting utilizing evidence-based practices.
- Not excluded from participating in Medicaid, Medicare, or any other federal or Michigan health care programs.ย
- No criminal history that would preclude employment under federal and state laws and regulations in a federal or state health program.ย
- Must support a trauma informed culture of safety to aid customers in their recovery process.
- Strong verbal and written communication skills.
- Child Mental Health Professional qualifications (as defined by the Michigan Department of Health and Human Services) is preferred.ย
- One year of computer experience โ Word, Excel and electronic medical record software preferred.
