Job description

Registered Nurse

Seeking a skilled and compassionate Registered Nurse to provide high quality, patient centered care in the home setting. This role focuses on supporting patients with disease management, medication education, and promoting safe and independent living. The ideal candidate is patient focused, detail oriented, and experienced in home health care delivery.

Location: Jackson, Clinton, Vicksburg, Bolton, Edwards, Raymond, Utica

Schedule: Monday through Friday, 8:00 AM to 4:30 PM

On Call/Weekends: Required 1 to 2 weekends per month

Weekly Hour Guarantee: 40 hours

Pay Rate: $51 to $53 per hour

Mileage Reimbursement: Yes, reimbursed between patient visits

Deliver skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans

Educate patients and families on disease management, medication regimens, safety precautions, and self care strategies

Perform comprehensive patient assessments and ongoing evaluations to track progress and adjust care plans as needed

Administer medications and treatments accurately and safely

Document all care provided in a timely and accurate manner using electronic medical records

Collaborate with interdisciplinary team members to ensure coordinated and effective patient care

Promote patient independence and quality of life through education and support

Active Registered Nurse (RN) licensure

Experience in home health or a similar clinical setting preferred

Familiarity with Homecare Homebase or PointCare strongly preferred

Strong clinical assessment and critical thinking skills

Ability to work independently and manage time effectively

Valid driver's license and reliable transportation
