Position Overview:


The SMI Case Manager plays a crucial role in supporting Alium Health’s patients throughout their healing process. This position involves developing and managing holistic, evidenced-based case management plans, acting as a community liaison, and working closely with patients and the care team to ensure effective and compassionate care.


Essential Job Functions:

  • Patient Support: Support patients throughout their entire healing process, ensuring they receive comprehensive care tailored to their needs.
  • Program Development: Assist in developing a holistic, innovative and integrated outpatient therapy program in collaboration with Alium Health providers.
  • Case Management:
  1. Evaluate patient needs and develop individualized holistic and integrated outpatient comprehensive care tailored to their needs.
  2. Regularly meet and check in with clients in person, over the phone, in their homes, or through telemedicine applications.
  3. Conduct intake and Annual Assessments
  4. Conduct Intake and Annual Health Risk Assessments
  5. Conduct Intake and updated At Risk Crisis Plans
  6. Ensure Case Management Services meet Mercy Care’s Industry Standards
  • Community Liaison:
  1. Act as a community liaison by developing and maintaining an external network of applicable community resources. Leverage these resources to best serve the patient.
  2. Coordinate patient care with in-patient hospital teams as indicated.
  3. Assist members with necessary housing applications/needs and attend necessary
  • Progress Monitoring: Monitor client progress and adjust case management plans as needed with guidance from Alium Health providers. Maintain well-organized and updated case management documentation after care team meetings.
  • Discharge Planning: Develop appropriate and empathetic discharge or closure plans when necessary, ensuring Smooth transition for the patient.
  • Literature and Best Practices: Stay up-to-date and well-versed in case management practices and literature related to the designated patient population.
  • Team Collaboration: Work closely with all Alium Health Team members involved in the patient’s plan. Provide regular updates and be knowledgeable about the entire integrated healthcare plan.
  • Patient Care: Consistently demonstrate sensitive, respectful and compassionate patient care. Complete assessments, treatment plans and updated patient paperwork under the supervision of a Behavioral Health Professional.
  • Meetings and Documentation: Actively participate in weekly supervision meetings and integrated Team meetings. Provide thorough and timely updates. File all patient documentation promptly.

Requirements:

Education and Experience Requirements:


Education: Bachelor's degree in social work, Psychology or a related field from an accredited college or university.

Experience: Minimum of 1 year of experience in social work, psychology, and/or case management.


Productivity Requirements: 108 units/week * 1 unit = 15 mins


Knowledge, Skills, and Abilities Required:


Knowledge of: Patient treatment plans, social work/psychological theory and practice case management procedures and documentation.

Technical Skills: Technologically savvy with the ability to use relevant software tools.

Teamwork: Ability to work well in a team environment, adapting to changing workflows and procedures with a positive attitude.

Interpersonal Skills: Strong interpersonal communication skills with the ability to work effectively with a diverse range of individuals and constituents.

Accreditation: Knowledge of related accreditation and certification requirements.
